Requesting Tutorial to create new offsets...

From what I have read on the forums in order to create the new offsets when a patch is placed you need to re-write the eqgame.ini. My question is do I compare the old MQ eqgame.ini with the new patch eqgame.ini? And basically what am I looking for?

After re-write is the new eqgame.ini placed in the EQ directory or the MQ directory?

If I am completely out of your world, plz explain to me how I go about creating my own offsets.
